Medical Records

To request a copy of your medical records, please complete BOTH PAGES of the attached form. Present the completed form and a copy of a valid driver’s license or government issued identification to the Health Information Management Department located on the 2nd floor of the main hospital. Fees are applied for medical record copies for personal use. Please allow 5-7 business days for requests to be completed.

Birth Certificates

Birth certificate information may be obtained by calling St. Mary's Birth Clerk at  (562) 491-9303. You may be referred to the L.A. County Hall of Records, Norwalk, CA which can be reached at (562) 462-2137.
