Before you are admitted to St. Mary Medical Center or receive outpatient care at the hospital, you will need to complete the registration process.

Inpatient/Outpatient Admissions

If you are being admitted to the hospital, please come to the Admitting Department in the Main Lobby.  Once you have completed the registration process, you will be escorted to your hospital room.

What to Bring:

• Photo ID
• Insurance card(s)
• Any forms, paperwork, pre-op labs, or x-ray results from your doctor's office  
• List of current medications
• Your healthcare advanced directive form if you have completed one.  (If you do not have an healthcare advanced directive but would like one, the nurse facilitator can provide you with information and a form.)

After business hours, and on weekends and holidays, please go to Emergency Room Admitting.
